Clark Consulting & Associates, LLC has been providing geriatric care management since 1992.  Located in the heart of the Peninsula, (in the San Francisco bay area), we serve private families by providing professional geriatric care management for older adults in San Mateo County, California.

We assist those involved in transitions arising from aging.  We serve the family and the elder through personalized guidance, collaboration with other professionals, and independence advocacy.

We find that the changes associated with process of aging can place a substantial burden upon the family and adult children to care for an older adult.  A professional geriatric care manager can bring peace of mind and a sense of order to the process.  

Among other things, we can:

• Conduct a care planning assessment to identify problems and recommend solutions.
  
• Screen, arrange, and monitor in-home help and a variety of other services (such as medical equipment, home repairs, gardening, groceries, etc.).
• Review financial, legal, or medical issues, and coordinate referrals to geriatric specialists.
• Provide crisis intervention and counseling.
• Act as a liaison to family members at a distance, monitor care and third-party services, and alert family members to problems or issues.
• Arrange for, and assist with, the moving of an older adult to or from a hospital, an assisted care home, a retirement home, a nursing home, or other facility.
• Arrange for and monitor therapeutic socialization, proper diet/meals, or alternative medicine or therapies, which might be appropriate or useful.
• Arrange, attend, and report on doctor visits, act as a medical advocate, and arrange for medications management.
• Provide education, guidance, referrals, counseling and support to the family and the older adult.
• In general, provide the advocacy that is so often needed in today’s institutionalized world.
